Mitte Februar 2009 erschien das nur als Download via Xbox Live (für PC und PS3 bis dato nicht verfügbar) beziehbare, 2 Episoden lange Add-on "GTA IV: Lost and Damned", das eine Parallelgeschichte erzählt, in der Nico Bellic, der Anti-Held des Hauptspiels, nur einen kurzen Gastauftritt hat, während der Spieler in die virtuelle Haut des Bikers Johnny schlüpft. Zur gleichen Zeit sorgte in Deutschland (und anscheinend auch z.B. im Vereinigten Königreich) ein Update für GTA IV über Xbox LIVE, das zum Betreiben von "Lost and Damned" erforderlich ist, für Erstaunen: plötzlich breitete sich in den eigentlich unzensierten Versionen keine Blutlache mehr unter dem Körper eines getöteten Gegners aus. Ebenso fehlten nun Blutflecke/Einschusswunden auf der Kleidung. Bei Inanspruchnahme einer Prostituierten konnte der "Service" nicht mehr gewählt werden, alle Sexanimationen wurden entfernt und die Kamera rotierte automatisch hinter das Auto und ließ sich währenddessen nicht mehr drehen.
