"Reaper of Souls" wurde im Rahmen einer Investorenkonferenz offiziell bestätigt. Die Erweiterung baut die Spielmechanik des Originals weiter aus und führt neben Feintuning auch gravierende Neuerungen ein. Dabei sind beispielsweise enthalten:
- Eine neue spielbare Charakterklasse, der Kreuzritter (vergleichbar mit dem 'Paladin' aus "Diablo II")
- Erhöhung des maximalen Charakterlevels von Stufe 60 auf 70
- Durch die zehn zusätzlichen Level erhalten alle alten Charakterklassen neue Fähigkeiten
- Ein zusätzlicher Abschnitt in 'Akt V'
- Ein neuer Spielmodus, der Abenteuermodus (erst zugänglich, nachdem man Akt V durchgespielt hat)
- Loot 2.0, ein komplett neu aufgestelltes Loot-System ohne Auktionshaus (war zuvor Teil von Patch auf v2.0)
- Paragon 2.0, welches das alte System aus "Diablo III" überarbeitet
- Die Schwierigkeitsgrade wurden überarbeitet, dazu gibt es neue zusätzliche Schwierigkeitsgrade
