Ende November 2011 verriet Hideo Kojima, dass es gegenüber dem PS2-Original keine neuen, exklusiven Inhalte geben, sondern das Spiel nur an die 3DS-Hardware angepasst würde.
Dies ist jedoch nur teilweise wahr: Zwar sind Areale, Gegner und Geschichte immer noch identisch, das sonstige Spielerlebnis (Gameplay, Interface, ...) wurde jedoch von „Metal Gear Solid: Peace Walker“ übernommen (auf dessen Engine diese Version basiert) und weicht deutlich von der Originalversion ab.
